A Year of Growth, Service, and Community Impact: Our 2025 Annual Report

CCACC Health Center is proud to release our 2025 Annual Report, reflecting on an year of tremendous growth and deepened commitment to the community we serve. We are thrilled to have supported thousands of residents this past year—2025 was especially meaningful as we celebrated the grand opening of our new 8,150‑square‑foot health center at 9298 Gaither Road. This expansion has allowed us to enhance access to care, strengthen our programs, lay the groundwork for future services, and really establish ourselves as a one-stop comprehensive health center.

Throughout 2025, our team provided care to 2,018 unique patients through more than 6,000 medical visits, offering comprehensive primary care, mental health services, and rehabilitation support. Beyond clinical care, we connected over 500 community members to essential resources that help overcome barriers related to language, insurance, and navigating the healthcare system.

This year also marked significant progress in our community outreach efforts. CCACC Health Center continued to host free health talks, workshops, and wellness events, empowering residents with practical knowledge and tools to improve their health. Our staff and volunteers attended numerous community events across the region, offering screenings, education, and support to promote overall wellness. These efforts reflect our ongoing mission to address social determinants of health and ensure that every individual has access to the care and information they need.

Our multilingual providers and staff deliver culturally competent care tailored to the needs of our patients, ensuring that everyone feels welcomed, understood, and supported. As we continue to grow, we remain committed to expanding services, deepening partnerships, and building a healthier, stronger community for all.
