AAPI Health Bridges Project
With support from the Maryland Department of Health, CCACC Health Center helps community members who face barriers to quality healthcare. We focus on social determinants of health (SDOH) — things like income, housing, education, and access to care.

Social Needs Assessment
Identify challenges and connect families to the right services.
Primary Care Access
Support for uninsured or underinsured individuals to see a doctor.
Benefit Enrollment & Referrals
One-on-one help signing up for government and community programs.
Health Education
Workshops and information to support healthier choices.
Food & Nutrition
Guidance and resources for families facing food insecurity.
Weight Management
Nutrition, exercise plans, and follow-ups for healthy living.

Since 2013, CCACC has partnered with Montgomery County to help seniors and low-income residents:
