Health Programs

One of our key missions is to reduce health disparities in our community. Thanks to grant support, CCACC Health is able to run several community health programs that bring care and resources to those who need them most.

PROMISE Camp

Provides a supportive experience for youth with special needs, helping them develop life skills.

PEARLs Program

Supports socially isolated adults in managing depression.

Village Program

Offers support and resources for dementia care.

STOP B

Provides hepatitis B screening, treatment, outreach, and prevention services.

Prevention for All

Promotes healthy living through preventive care and education.

Chronic Care Management

Provides support to help patients with chronic conditions manage pain and improve quality of life.

Overdose Response Program

Certified as an ORP by the Maryland Department of Health, provides training in naloxone use to prevent overdose deaths.

Benefits Enrollment

Assists individuals with enrolling in programs that address social determinants of health (SDoH).
