Social Determinants of Health (SDOH)
AAPI Health Bridges Project
Funding support from the Maryland Department of Health (MDH), Office of Minority Health and Health Disparities enables CCACC Health Center to provide comprehensive services to community members who face barriers to accessing quality healthcare due to social determinants of health (SDOH). Social determinants of health refer to a range of social, economic, and environmental factors that affect an individual's health, such as education level, living conditions, and economic status.
Benefits Enrollment Center

Benefits and Enrollment
Navigating the federal healthcare policies may be challenging for previously uninsured individuals. The Affordable Care Act (ACA), passed in 2010, requires that every individual meet the Minimum Essential Coverage (MEC) through Marketplace plans, job-based plans, Medicare, Medicaid, or CHIP. Those who remain uninsured will face a penalty proportional to income. To avoid paying these penalties and missing opportunities to enroll in federally-funded healthcare plans, we offer services to understand eligibility and enrollment.
Most of our services are free; we welcome anyone who volunteers or donates so we can continue to help people who need them. Reach out to us for more information.
