Healthy Communities Funds

CCACC Health Center received a Healthy Communities Fund from Montgomery County



CCACC Health Center under the Chinese Culture and Community Service Center (CCACC) has recently received funding from the " Asian American Health Initiative (AAHI)" of the Montgomery County government. We will provide more health-related services to our community from November this year to June next year. During the grant period, CCACC Health Center will provide the community with disease prevention services, monthly health talks, chronic disease (diabetes, hypertension, chronic pain, etc.) workshops, as well as youth mental health first aid training and other services. The following is our tentative arrangement for the services:

  • January 2023: Diabetes Self-Management Program (DSMP)

Time: 3:00-5:00 pm (Every Thursday from 3-5 pm for 6 weeks)

  • 1/14/2023: Youth Mental Health First Aid Training

  • February 2023: Chronic Pain Self-Management Program (CPSMP)

Time: 3:00-5:00 pm (Every Thursday from 3-5 pm for 6 weeks)

  • 3/18/2023: Youth Mental Health First Aid Training

  • April 2023:  Building Better Caregivers (BBC)

Time: 3:00-5:00 pm (Every Thursday from 3-5 pm for 6 weeks)

  • May 2023: Disease Prevention Month (Free Blood Work)

We also provide patients with personalized chronic disease management services and rehabilitation services (including physical therapy and occupational therapy) for patients with chronic pain.Our psychiatristst and mental health counselors provide individual services such as medication and counseling for patients with psychological problems. We also continue to provide the PEARLs project (Program to Encourage Active and RewardingLivesS) for the isolated elderly living in the community and provide systematic care services for the isolated elderly who have minor depression. We hope to create a healthier community through these services.


In partnership with the Montgomery County Public Health Department, in addition to providing COVID-19 testing and vaccination services for the community, we also insist on providing primary care, rehabilitation service, and mental health services. We continue to provide primary care to uninsured, low-income Montgomery County residents in our community, and we also provide medical services to insured residents. CCACC Health Center now accepts Medicare, Medicaid, and commercial insurance such as Care First, UnitedHealthcare, Aetna, Cigna, and Blue Cross Blue Shields.
